Extension of intestinal exeresis, management of the ends, restoration of digestive continuity and peritonization

Se señalan las diferentes longitudes de resecc10n en las extirpaciones ileocólicas por cáncer colónico derecho. Se establecen las maniobras quirúrgicas que conducen a un correcto manejo de los cabos colónico e ileal, su preparación, la coprostasis y su sección siguiendo normas anatomofisiológicas y bacteriológicas. El restablecimiento de la continuidad es
analizado según los tipos de anastomosis: terminoterminal, laterolateral y terminolateral. Asimismo se describen las variantes de más interés en cada tipo, planteándose
los argumentos a favor de cada método. Son estudiados los tipos de sutura intestinal en uno y dos planos, sus ventajas e inconvenientes y el material a utilizar.
Se señala la necesidad imperiosa del cierre de la brecha mesentericomesocólica. Deben evitarse brechas peritoneales en la peritonización parietal, pues pueden extrangularse
asas. Pueden dejarse superficies planas deperitonizadas, las cuales no dan complicaciones. Se considera necesario drenar ef retroperitoneo y hacer intubación nasogástrica
en las colectomías derechas.
