Abstract

The author analyzes the historical foundations of the so-called right colectomy, the approach to the tumor, its mobilization and the extent of the intestine excision.
He points out how late he entered the current technique, the concept of lymph node dissection. Three aspects of the surgical technique are studied: the incision, exploration and
mobilization of the right colon. On this last point, the two ways of decoupling are analyzed: from the outside to the inside, and in the manner of J. Gosset, from the top to the top.

And finally, the error is pointed out, from the point of view of the pathology, of treating the piece to be resected in a centripetal manner, from the tumor to the pedicle of the hemicolon, without prior blockage of its blood and lymphatic drainage pathways.
